PRC Board Exam Requirements 2026: Complete List by Profession
What documents do you need to apply for a PRC board exam in 2026? Complete requirements list for nursing, criminology, LET, pharmacy, medtech, and agriculture.
General Requirements (Almost Every PRC Board Exam)
These documents are required across most PRC licensure examinations. Prepare all of them regardless of which profession you are applying for.
1. Printed Application Form
Fill this out through the PRC LERIS portal at online.prc.gov.ph. Once submitted and approved, print it and bring it to your document verification appointment.
2. PSA-Authenticated Birth Certificate
Must be from the Philippine Statistics Authority — not just a Local Civil Registry copy. Order at serbilis.psa.gov.ph. Processing takes time so request early.
3. Official Transcript of Records (OTR) with School Dry Seal
Your OTR must be issued directly by your college or university with the registrar's signature and school dry seal. Some schools require 2 to 3 weeks to process OTRs — request early.
4. Certificate of Registration or Diploma
Some boards require a photocopy of your diploma; others require a Certificate of Graduation. Check your specific profession's requirements.
5. Recent 2x2 ID Photos
6. Valid Government-Issued ID (with photocopy)
Accepted IDs: Passport, Driver's License, PhilSys National ID, UMID Card, Voter's ID, Postal ID, NBI Clearance.
Bring the original and one photocopy of both front and back.

How to Prepare Your Documents Without Stress
